Hotfix for custom accountability partners.

• Custom partner image changes now persist — when editing an existing partner.
• Optional pose images can be removed — and correctly fall back to the main portrait.
• Visible custom portraits refresh — after in-place image edits.

I've tried to ship this with guardrails so it isn't harmful but still a helpful focus companion. For this reason, some characters might not always come through as strong as expected — a deliberate tradeoff. Hope you can still get the character you need to focus and do some cool stuff with it :D
